All Shore Symphonic Band Application Form

Applications must be postmarked by February 7, 2018
- Applications postmarked on or after February 7 will be considered late and incur a $10 late fee per instrument
- Applications postmarked after February 14, 2018 will not be accepted
Please note all registrants will be screened for eligibility by their sponsoring director
- A completed signature form, with accompanying fees, must be submitted for registration to be complete
$15.00 – Audition Fee submitted with each signature page
$10.00 for each additional instrument
Only one registration form is necessary for those auditioning on multiple instruments; You do NOT need to fill out this form more than once
Accepted Students will pay an additional $20 participation fee due by the second rehearsal
- Students will not be allowed to perform at the All Shore Concert until this fee is paid
All signature forms and fees must be postmarked by February 14, 2018 to be accepted
The sponsoring director will forward all signature forms, materials and one check or money order to the auditions chairperson
